The Art of Bedside Diagnosis
In an era dominated by imaging and laboratory automation, the stethoscope, the percussion hammer, and focused clinical palpation remain irreplaceable. The Weekly Bedside Case Conferences train students to identify physical exam signs accurately before reviewing ancillary tests.
Standardized Case Conference Protocol
Each Saturday morning session follows a structured 90-minute pedagogical format:
- 1.Uninterrupted 10-Minute Patient History: A designated medical student elicits the chief complaint, chronologic history of present illness, and systematic review.
- 2.Targeted Physical Examination Demonstration: Senior registrars guide students on subtle inspection findings, cardiac murmurs, pulmonary crackles, and abdominal masses.
- 3.Problem Representation Formulation: Synthesizing raw data into a concise one-line clinical summary.
- 4.Differential Diagnosis Defense: Formulating 'Must Not Miss', 'Most Likely', and 'Uncommon' diagnoses.
Clinical Competencies Emphasized
Cardiovascular ExaminationCardiology
- Distinguishing innocent systolic murmurs from aortic stenosis (radiation to carotids, slow-rising pulse).
- Identifying JVP elevation, abdominojugular reflux, and differentiating arterial vs venous neck pulsations.
- Detecting pericardial friction rub and third heart sound (S3 gallop) in acute heart failure.
Neurological LocalizationNeurology
- Precise cranial nerve examination (diplopia differentiation, facial nerve upper vs lower motor neuron patterns).
- Systematic sensory level localization in paraparesis and spinal cord lesions.
- Differentiating cerebellar ataxia from sensory ataxia using Romberg's test and heel-to-shin coordination.
